Milan's German defender Malick Thiaw drew first blood at Madrid's Santiago Bernabeu Stadium, scoring an early opener against the Whites.
Real Madrid's Brazilian superstar Vinicius Junior, who was brought down in the penalty area, scored from a penalty kick to equalize in the 23rd minute.
Milan took the lead again when Spain's Alvaro Morata, a former Real Madrid forward, scored an empty goal in the 39th minute.
The Italian team's Dutch midfielder Tijjani Reijnders added a third goal in the 73rd minute to cement the 3-1 win.
Milan took a historic win as they beat Real Madrid for the first time in 15 years.
Milan’s goalkeeper Mike Maignan was named the man of the match after he made nine saves.
Following the win, Milan have six points in four matches and improved to the 18th spot.
Carlo Ancelotti's Real Madrid have six points as well as the Whites fell to 17th place.
In the other match of the night, Sporting Lisbon beat Pep Guardiola's Manchester City 4-1 in a home match to secure a memorable win.
Man City's English star Phil Foden scored the opening goal in the fourth minute at Lisbon's Jose Alvalade Stadium.
But the hosts' Swedish forward Gyokeres was on fire and scored a hat-trick as Sporting came from behind.
Uruguayan midfielder Maximiliano Araujo added a goal for Sporting.
Erling Haaland, Manchester City's Norwegian superstar, missed a penalty shot in the second half that encouraged the hosts.
Gyokeres was named the player of the match for scoring three goals, including two penalties.
Sporting increased their points to 10 in four matches and is in the second position in the Champions League led by England's Liverpool.
Liverpool beat Germany's Bayer Leverkusen 4-0 in a home match at the Anfield Stadium. The Reds are perfect with 12 points in the Champions League standings.
