Barcelona began the game with high press despite playing away and missed several scoring chances at Diego Armando Maradona Stadium.
After taking Pedri's pass, Polish striker Robert Lewandowski ended his four-match scoreless streak in the competition with a low drive and scored in the 50th minute.
Napoli's Nigerian striker Victor Osimhen beat his marker and leveled the score with a low shot in the 70th minute.
Argentine attacker Giovanni Simeone of Napoli missed the chance to score a late winner in the 89th minute.
The second leg will be played on March 12 at Lluis Companys Olympic Stadium.
Portuguese club Porto beat English Premier League side Arsenal 1-0 with a late goal in the other UEFA Champions League Round of 16 first leg match of Wednesday.
Porto's Brazilian forward Galeno hit the woodwork and then put the returning ball wide and made a double miss in the 21st minute at Dragao Stadium.
Galeno made amends for his earlier mistake in the 94th minute when he beat Arsenal goalie David Raya with a screamer to score the winning goal for the hosts.
The second leg will be played on March 12 at Emirates Stadium.
